If you choose to use the downloadable form, please follow the instructions provided on the form for submitting the completed authorization.
Processing Time
Medical records are often forwarded within approximately two weeks. However, please note that federal and state regulations allow healthcare providers up to 30 days to respond to written requests for medical records. Thank you for your patience while the request is being processed.
