If you would like to obtain a copy of your medical records from our practice, requests may be submitted electronically through HealthMark’s Request Manager.
You will be asked to enter your email address. HealthMark will then send a secure link to your email that will take you directly to your Request Manager Dashboard.
Please note:
- The secure link sent by email expires after 15 minutes, so it should be opened as soon as it is received.
- Once logged in, select “Submit Request” from the menu options.
- Complete all required fields to provide authorization directly to HealthMark.
Your request will be processed by HealthMark, and you will receive a notification by mail or email once your records are complete and available for download.
